In late 2024, Centers Health Care, the owner and operator of four major nursing homes in New York, was ordered to pay $45 million to settle claims that they embezzled more than $83 million in Medicare and Medicaid funding from their facilities. As of early 2026, there are four one-star nursing homes in Rockland County. These incidents, including a January 2026 fire at the Friedwald nursing home in New City, show an overall management issue and other underlying problems with nursing facilities in Rockland County.
In November 2025, the owner of the Evergreen Court Home for Adults Nursing Home in Spring Valley was ordered to pay more than $700,000 in civil penalties for building code and safety violations after a fire at the facility. The fire killed two people—one resident and one firefighter—and the owners then left the building as an unsecured, burned-out site, repeatedly ignoring the city’s orders to clean it up. Earlier the same year, the town of Haverstraw filed a lawsuit against the owners of the Green Hills Adult Care and Assisted Living Facility for alleged supervision failures that caused chaos in the local town.
Northern Manor Geriatric Center Inc. (Nanuet)
Northern Manor Geriatric Center Inc. in Nanuet is a 231-bed nursing facility with nonprofit corporate ownership. The facility has an overall rating of 1 star, a health inspection rating of 1 star, and a staffing rating of 1 star. It reported a total staff turnover rate of 24.3% in 2024, with a 50% turnover rate for registered nurses, and had 10 health deficiencies cited that year.
Tolstoy Foundation Rehabilitation and Nursing Center (Valley Cottage)
The Tolstoy Foundation Rehabilitation and Nursing Center in Valley Cottage is a 96-bed nursing facility with nonprofit corporate ownership. The facility has an overall rating of 1 star, a health inspection rating of 1 star, and a staffing rating of 1 star. There were 21 reported health deficiencies at the facility at its latest inspection in September 2025.
Nyack Ridge Rehabilitation and Nursing Center (Valley Cottage)
The Nyack Ridge Rehabilitation and Nursing Center in Valley Cottage is a 160-bed nursing facility owned by a for-profit entity. The facility has an overall rating of 1 star, a health inspection rating of 1 star, and a staffing rating of 3 stars. It had the highest staff turnover rate of any one-star-rated facility in Rockland County, with 31.3% total turnover and 51.9% registered nurse turnover. A January 2025 inspection found 21 health deficiencies, and it paid more than $39,000 in fines for regulatory violations.

Northern Riverview Health Care Inc. is a 182-bed nursing facility in Haverstraw with nonprofit corporate ownership. The facility has an overall rating of 1 star, a health inspection rating of 1 star, and a staffing rating of 2 stars. A February 2024 inspection found 19 health deficiencies.
