In August 2025, Attorney General James secured a $12 million settlement against the owners of Van Duyn Center for Rehabilitation and Nursing in Syracuse — a facility that had been under investigation for years of neglect, abuse, and financial fraud. Of the $12 million, $2 million was directed to New York’s Medicaid program as restitution, while $10 million was placed into a Resident Care Fund to fund facility-wide reforms overseen by independent monitors. The settlement requires Van Duyn to appoint an Independent Health Care Monitor and a Chief Compliance Officer, and prohibits the owners from closing or selling the facility for at least five years. The owners are also subject to a $1 million penalty if they fail to maintain the staffing levels recommended by the independent monitor after the settlement period ends. The Van Duyn case was the sixth nursing home that Attorney General James held accountable for resident mistreatment and neglect as part of an ongoing statewide enforcement effort.
