About Graduate Post Acute
Graduate Post Acute is a 150-bed nursing home in Philadelphia owned by Genesis Healthcare. After the facility’s most recent health inspection in November 2024, the facility received 33 citations–more than three times greater than the state and national average. The Department of Health & Human Services’ report found evidence of financial abuse, stating that Graduate Post Acute failed to ensure residents were free from misappropriation and exploitation of property related to authorized use of residents’ funds.
The report also included evidence of pests in the facility, with thirteen mouse and other pest sightings in the first half of October 2024. Graduate Post Acute is also reported to have a high percentage of long-stay residents with pressure ulcers and a high rate of long-stay residents whose ability to walk independently worsened after receiving treatment at the facility.
