A recent preliminary report by the National Transportation Safety Board (NTSB) has provided new insights into the deadly explosion that rocked the Bristol Health and Rehab Center in Bristol Township, Bucks County. On December 23, 2025, a blast claimed the lives of three individuals and injured 20 more, leaving a trail of devastation at the nursing home.
According to the NTSB, the explosion occurred after a gas leak was reported earlier that day. The facility’s maintenance director reached out to PECO, the energy supplier, at 11:00 a.m. regarding a natural gas odor detected in the basement and first-floor hallway. Despite a technician arriving by 11:50 a.m. and identifying the leak on a meter set valve, the situation remained unresolved until after the explosion, which occurred at approximately 2:15 p.m.
Investigation into Gas Leak and Personnel Response
The investigation has revealed that the explosion was preceded by escalating gas odors throughout the building, affecting even the second floor shortly before the blast. The report also highlights that the two Exelon technicians dispatched to address the issue had less than a year of experience in their current roles, raising questions about personnel qualifications and emergency response protocols. Exelon, the parent company of PECO, is now under scrutiny for its handling of the situation.
The NTSB continues to investigate the incident, with a focus on the physical evidence collected, as well as Exelon’s pipeline safety management system and emergency response procedures.
Fatalities and Injuries
The explosion killed three individuals: residents Patricia Mero, 66, and Ann Reddy, as well as 52-year-old nurse Muthoni Nduthu. Many others were injured, and roughly 180 people were inside the facility at the time of the explosion.

The company outlined several safety improvements, including relocating indoor meters to outdoor locations, updating procedures for emergency responses, and enhancing employee training.
